APPOINTMENTS AND CANCELLATION POLICY
Patients are seen by appointment only. To request an appointment, please contact us at (302) 644-7201.
If you need to cancel or reschedule your appointment, please give us as much notice as possible, but no less than 24 hrs. When you schedule an appointment with one of our providers, that time is reserved for you. When you fail to show, or cancel at the last minute, it is not only a financial loss to the practice, but it is a time slot we could have given to another patient, perhaps someone who was sick and needed to be seen. For this reason, we may charge a fee of $25 for appointments missed or cancelled with less than 24 hours notice.
If you are a new patient to our practice, we welcome you. Please print the forms and fill them prior to your appointment. This will save you time by not having to complete these forms while in office.
What to bring for your appointment
- Insurance cards
- Referral (if required)
- Any required co-payments
- List of all prescription and non prescription medications (if you do not have an updated list then bring medicine bottles) Pertinent medical and surgical history information
If you are being seen for:
Lung problems please bring with you any radiological studies like Chest X-ray, CT scans (actual film or disk and not just the report), lab work, PFTs (breathing test) and any other pertinent testing to your first appointment.
Sleep Problems please bring results of any previous sleep studies. If you have a CPAP/BiPAP machine, please bring the machine along with the mask that you use.
Diabetes please bring your recent labs, Blood sugar meter
Thyroid problems please bring recent labs, any thyroid ultrasounds, scans.
Osteoporosis please bring results of DEXA scan and any recent labs.
For refills of prescriptions provided by our providers, please call your pharmacy and ask them to send us the prescription refill request electronically or via fax. You can also leave message on our prescription mailbox. Please specify your name, date of birth, pharmacy name and location, name of medication, strength, 30 or 90 day supply. Please allow 2 business days for refill of prescriptions. Please note: Doctor on call will not authorize refill at night or on the weekend.
